Eligibility requirements for citizens
- 18 years of age to 65 years of age
- Individuals name on Lease, Lot rent or Mortgage
- Invasive Major Surgery to Remove Cancer
- Doctors Note Validating recovery time of 6,9,12 weeks to heal from surgery.
- Validation Letter from landlord or mortgage company on your monthly payment.
You'll need to apply 14 days before your surgery or 14 days after your surgery. We will write a check to the appropriate landlord or mortgage company. If your recovery is 6 weeks, the financial resource will cover a month and half of your rent or mortgage, 9 weeks / 2 months, 12 weeks / 3 months.
